| Welcome to
Arthritis and Rheumatology Clinics of Kansas (ARCK). We look
forward to getting to know you while assisting you in the
management of your rheumatic disease. We want you to know
that we have a high level of commitment to providing quality
care in an efficient and compassionate manner. In doing so,
it is our desire to run our office as efficiently as
possible, and we want to communicate some guidelines to you
that will allow us to achieve this goal together. |

New Patient Information Form |
| |
| There
are several forms that we need to have our new patients fill
out before their first appointment. In order to make sure
that you have plenty of time to complete them, and to
provide the best possible legibility, we have made these
forms available here for you to download and complete. |

In
order to view and fill out these forms you must have
Adobe Reader installed on your
computer. This software is free - if you do not have it on
your computer,
click here to go to the Adobe web site where you may
download and install the software. |

| After
you click on the form you wish to work with, it will be
displayed in a separate window. The places on the form where
you can type information are highlighted. If they are not
highlighted, click the "Highlight Fields" button in the
upper right corner of the window. You may click with
your mouse to select the first field to fill out. After
this, you may continue through the highlighted areas on the
form by pressing the "Tab" key. |

After
you have completed the form, you must print it.
You cannot save the completed
form on your computer, so you must print it before closing
the window. As soon as you close the window,
all the information you have entered will be lost. If you
have not printed it, you will have to start over. |
